A 12-month minimum term applies. Cancellation requires 30 days’ written notice to help.care@mysanitas.com. An early-termination fee equal to two months of membership applies. It is waived if the member enrolls in a Florida Blue Individual or Medicare product and designates Sanitas as their primary care provider, with the required proof.
